The Benefit information at the links bellow was from a document obtained by CEP Local 715M from our local Human Resources department (document obtained in April, 2010 but the document itself is dated February 2006), scanned, then converted into text via OCR (optical character recognition), then converted and formatted into html as well as  manually checked for errors in the conversion process.  Although great care has been taken to process the original document accurately, the information is presented AS-IS, and should be considered an information guide only with the owness on the user to verify coverage with our insurance provider.
